Alamo Hospice in Waco, TX is seeking a PRN Nurse Practitioner to join our growing hospice team. You will provide compassionate nursing care, determine eligibility for home-based services, and coordinate with physicians and the interdisciplinary team.

This role offers flexible hours, the opportunity to work in patients' homes across Waco and surrounding counties, and competitive pay with a supportive, patient-centered culture.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a Nurse Practitioner in the state of practice.
• Minimum of two years of varied work experience as a professional nurse.
• Current CPR Certification.
• Demonstrated nursing clinical judgment and skills.

Responsabilités

• Determine patient eligibility for care based on State/Federal guidelines.
• Collect data on physical, psychological, social, and spiritual needs.
• Coordinate admission/recertification with physician and team.
• Utilize resources to meet patient needs.
• Provide regular visits to reassess eligibility and update the care plan.
