Job summary

Affinity Hospice is seeking a Nurse Practitioner to join our fast-growing, nationally recognized team in Virginia. The NP provides hospice services under the state Nurse Practice Act, evaluates patient histories, diagnoses conditions, manages health problems, prescribes treatments, and collaborates with patients and healthcare practitioners.

They visit patients, perform assessments, and evaluate hospice eligibility.

Qualifications

  • Master’s degree in nursing or related field from an accredited college/university (preferred).
  • Minimum of one year of Nurse Practitioner experience.
  • Hospice or palliative care experience (preferred).
  • Current Nurse Practitioner licensure in the state.
  • CPR certification.
  • Accreditation from an approved certifying body for advanced practice nursing, as required by the state.
  • Certification in a specialty area (preferred), such as Hospice and Palliative Nursing (CHPN).
  • Valid driver’s license and access to insured, reliable vehicle as required.

Responsibilities

  • Provide medical care, palliate, and manage terminal illness under physician supervision.
  • Complete history and physicals, order/interpret lab tests, and document findings.
  • Develop individualized care plans, prescribe meds, obtain consultations, and coordinate hospice services.
  • Complete hospice face-to-face visits to assess eligibility for benefit periods.
  • Provide clinical information for eligibility and recertification decisions.
  • Collaborate with hospice medical director on patient eligibility for certification/recertification.
  • Coordinate patient care with the interdisciplinary team and other practitioners.
  • Ensure symptom control through team collaboration and protocol adherence.
  • Review and develop treatment protocols based on literature with physician input.
  • Consult with the physician when the plan of care is outside standard practice.

Position Summary:

A Nurse Practitioner is a registered professional nurse who provides hospice services under the state Nurse Practice Act, collaborating with a physician. They evaluate health histories, diagnose conditions, manage health problems, prescribe treatments, and collaborate with patients and healthcare practitioners. They visit patients, perform assessments, and evaluate hospice eligibility. They use community resources to plan services and meet individual and family needs.

Minimum Qualifications:
  • Master’s degree in nursing or a related field from an accredited college or university, or an international equivalent (preferred).
  • Minimum of one (1) year of experience as a Nurse Practitioner.
  • Hospice or palliative care experience (preferred).
  • Current Nurse Practitioner licensure in the state.
  • CPR certification.
  • Accreditation from an approved certifying body for advanced practice nursing, as required by the state.
  • Certification in a specialty area (preferred), such as Hospice and Palliative Nursing (CHPN), Pediatrics, or Geriatrics.
  • Valid driver’s license and access to an insured, reliable vehicle in compliance with state and/or organizational requirements.
Key Responsibilities:
  • Providemedical care, palliates, andmanagesterminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician.
  • Complete history and physical examinations, by evaluating the patient’s medical condition and health history,orderingand interpreting results from laboratory and diagnostic tests or procedures, diagnosing health conditions and documenting the findings in the patient’s medical record.
  • Manage health conditions and terminal illnesses by developing an individualized plan of care, prescribing medications and/or treatments, obtainingconsultationsor making referrals and coordinating hospice care services.
  • Complete the hospice face to face visit requirements prior to the third andsubsequenthospice benefit periods to assess hospice eligibility based on the hospice conditions of participation and the local coverage determinations.
  • Provide clinical information to the hospice medical director for interpretationforeligibility for hospice recertification.
  • Make recommendations to the hospice medical directorregardingpatient eligibility for certification and recertification for hospice care.
  • Reviewclinical information, coordinates and oversees patient care in consultation with the hospice interdisciplinary team and other healthcare practitioners.
  • Assure that patients receiveappropriate measuresto controlsymptoms,through collaboration with interdisciplinary team members.
  • Review and develop protocols for treatment and propose options for interventions based on literature in collaboration with the collaborating physician.
  • Consultthe collaborating physician when the patient’s plan of care is outside standardized practice and protocols.
  • Communicate and collaborate with the interdisciplinary group to create,reviewand revise the patient’s plan of care.
  • Consult with the patient’s attending physician, the hospice medicaldirectorand other healthcare practitionersregardingthe patient’s eligibility for hospice care, ongoing careneedsand medical management related to the patient’s terminal condition.
