Reliant at Home is seeking a PRN Hospice Nurse Practitioner to cover Teague and surrounding areas. Hospice experience preferred; Home Care Home Base EMR experience is a plus.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current RN license and Driver's License.
• Two years' RN experience in a clinical setting; hospice preferred.
• CPR certification; read/write English.
• Reliable transportation.

Responsibilities

• Perform initial and ongoing comprehensive assessments of hospice patients.
• Determine eligibility and level of care for hospice services.
• Develop and revise the plan of care; ensure quality and continuity.
• Educate patients and families on disease, end-of-life care and self-care.
• Coordinate with the IDG; provide staff training as needed.
• Document progress notes and clinical outcomes.
• Communicate changes in patient needs to the physician and team.
