Job summary

Heritage Hospice is seeking a skilled Nurse Practitioner to perform required Face-to-Face encounters for hospice patients approaching benefit period recertification. You will assess prognosis, disease progression, and symptom burden, collaborating with IDG for accurate decisions.

Join a dedicated team supporting patients in homes and facilities across New Mexico, with travel within the service area and a focus on compliant documentation and clinical leadership communication.

Qualifications

• Current and unrestricted NM NP license.
• National NP board certification (FNP/AGNP).
• At least 1 year clinical experience; hospice/palliative/geriatric preferred.
• Understanding of hospice eligibility criteria and disease trajectories.
• Strong communication, interpersonal and clinical assessment skills.
• Reliable transportation and willingness to travel across assigned territory.

Responsibilities

• Conduct Face-to-Face encounters for patients entering their 3rd benefit period and beyond.
• Assess and document terminal prognosis, disease progression, functional decline, and symptom burden.
• Collaborate with hospice physicians, RNs, social workers, and the IDG to support accurate recertification decisions.
• Provide clinical insight and education to staff regarding disease trajectories, eligibility criteria, and hospice best practices.
• Maintain timely, compliant documentation aligned with Medicare and state regulatory standards.
• Communicate findings effectively to physicians and clinical leadership.
• Travel within assigned New Mexico service area to visit patients in homes, assisted living facilities, SNFs, or other care settings.
