Job summary

Three Oaks Hospice is seeking a Nurse Practitioner to join our team on a PRN basis. You will collaborate with physicians and the interdisciplinary group to deliver high-quality palliative care for chronically ill patients and families across Gwinnett, Forsyth, and DeKalb counties in Georgia.

The role emphasizes patient education, care planning, and coordination with other IDG members. A current RN license, APN credential, CPR certification, and hospice experience are preferred for this position.

Qualifications

• Must hold an RN license and APN credential with current CPR certification.
• Experience providing palliative or hospice care is preferred.
• Strong communication and problem-solving abilities are essential.

Responsibilities

• Collaborates with the IDG to evaluate and plan palliative care for terminal patients.
• Assist in developing patient/family plans of care to assure quality care.
• Educate patients and families about disease processes and end-of-life care.
• Visit patients/families according to the plan of care and needs.
• Coordinate with physicians on palliative care decisions and interventions.
• Provide education to IDG members on palliation as needed.
