Three Oaks Hospice is seeking a Nurse Practitioner to join our team on a PRN basis. You will collaborate with physicians and the interdisciplinary group to deliver high-quality palliative care for chronically ill patients and families across Gwinnett, Forsyth, and DeKalb counties in Georgia.

The role emphasizes patient education, care planning, and coordination with other IDG members. A current RN license, APN credential, CPR certification, and hospice experience are preferred for this position.

POSITION SUMMARY

The Nurse Practitioner works in collaboration with a physician and other palliative care team members to deliver palliative care and related health care services to chronic and serious ill patients and families.

Essential Duties

• Collaborates with the IDG in assuring appropriate evaluation and palliative treatment of terminal disease.
• Consults with and assists in the IDG's development of patient/family plan of care to assure quality care is provided.
• Consults with and educates the patient/family regarding disease process, self-care techniques, end-of-life care. Provides training to other staff as needed.
• Performs and documents visits to patient/family based on plan of care and patient/family need.
• Interacts with attending and consulting physicians as necessary regarding palliative care, in collaboration with hospice physician.
• Consults with and provides education to other IDG members on a prn basis for clinical issues relating to palliation.
• Actively participates in the IDG. Communicates effectively, timely, and appropriately at all times.
• Informs physicians and other personnel of changes in the patient's needs and outcomes of intervention, while evaluating patient/family response to care.

Qualifications

• Graduate of an accredited School of Nursing and accredited Nurse Practitioner Program.
• Current state RN and APN license and CPR certification.
• Minimum 2 years' experience as an APN clinical care setting preferred; hospice and home health preferred.
• Certification in a specialist area preferred; (e.g., Hospice and Palliative Nursing (CHPN) or Geriatrics)
• Excellent observation, verbal and written communication skill, problem solving skills, nursing skills per competency checklist.
• Reliable transportation, valid driver's license, and current auto liability insurance coverage.
