Job summary

Three Oaks Hospice, Inc. is seeking a compassionate Registered Nurse to administer skilled nursing care for patients in their homes and coordinate with a multidisciplinary team. The role covers Gwinnett, Forsyth, and Dekalb Counties, focusing on high-quality hospice care and family support.

The ideal candidate holds an accredited nursing degree, an active RN license, CPR certification, and at least two years of RN experience in clinical settings; hospice/home health experience is preferred.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ experience as an RN in a clinical care setting; hospice or home health experience preferred.
• Current CPR certification.
• Proficient skill using a computer; Microsoft Office experience preferred.
• Strong interpersonal skills for patient and family support.
• Demonstrate skill in nursing practice and ability to cope with family emotional stress.

Responsibilities

• Administer skilled nursing care for clients in their place of residence.
• Coordinate care with the interdisciplinary team, patient/family, and referring agency.
