Job summary

Three Oaks Hospice is seeking a PRN Registered Nurse to administer skilled nursing care for hospice patients at their homes in Cobb, Fulton, Clayton, Henry, and Fayette counties. Interviews will be conducted on-site at our Villa Rica office as part of joining our team.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license
• Two years’ experience as an RN in a clinical care setting; previous hospice or home health experience preferred
• Current CPR certification
• Proficient skill using a computer is required; Microsoft office suite experience is preferred
• Strong interpersonal skills and ability to provide effective support to the patient and family
• Demonstrate skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles.
• Strong communication and organizational skills and high-standard of ethics and compliance

Responsibilities

• Initial and ongoing comprehensive assessments of the impact of terminal disease on patients’ needs and ADLs.
• Implement the individualized POC and revise the plan as necessary.
• Educate patient/family regarding disease process, self-care, end-of-life care; provide training to other staff.
• Initiate appropriate preventive and rehabilitative nursing procedures.
• Prepare clinical and progress notes showing progress toward goals.
• Coordinate all patient/family services with the IDT.
• Refer to other services as needed using case management.
• Inform physician and others of changes in patient needs and outcomes.
• Determine scope and frequency of services based on acuity.
• Assess caregiver ability to meet patient needs.
• Communicate information using current organization tools.
• Apply admission and re-certification criteria for hospice care.
• Perform on-call responsibilities and provide on-call service as assigned.
• Other duties as assigned.
