Job summary

Elara Caring in Toledo, OH is seeking a dedicated Hospice Nurse Practitioner (PRN/Part-time) to join our home-based care team. You will conduct routine and emergency face-to-face visits, assess needs, and collaborate with physicians and the interdisciplinary care team to manage symptoms.

The role emphasizes bedside palliative care, education for patients and families, and timely documentation in CMS-compliant records.

Qualifications

• Current NP license in the practice state.
• Master's degree in nursing and national NP certification.
• 1+ years of clinical nursing experience, hospice/palliative care preferred.
• DEA certification if state allows prescribing controlled substances by APPs.

Responsibilities

• Complete routine and emergency face-to-face recertification visits for hospice patients.
• Assess patient needs, document findings, and collaborate with physicians and the care team.
• Provide bedside palliative care, education, and support to patients and families.
• Triage patient needs to prevent hospitalizations; maintain CMS-compliant records.
• Collaborate with leadership to coordinate quality patient care and training.
