Job summary

Elara Caring seeks a Hospice Nurse Practitioner (PRN) in Massachusetts to deliver compassionate home-based hospice care. You will perform admissions, recertifications, and ongoing symptom management in collaboration with physicians and the interdisciplinary team.

The ideal candidate has NP licensure in MA, a master’s degree, and at least 1 year of clinical nursing experience, preferably in hospice/palliative settings.

Qualifications

• Current licensure as a Nurse Practitioner in the state of practice and ongoing licensure compliance.
• Master’s degree in nursing and certification from a national NP licensing board.
• 1+ years of clinical nursing experience.
• Maintain active DEA certification if state allows prescription of controlled substances by APPs.
• Ability to frequently lift up to 50 pounds and assist with patient transfers and equipment.
• Valid driver’s license or reliable transportation to travel between patient homes and work locations.
• 1+ year post-graduate clinical nursing experience in hospice/palliative care preferred.
• Prior experience in Family Medicine, Geriatrics or Psychiatry preferred.

Responsibilities

• Complete routine and emergency face-to-face recertification visits for hospice patients.
• Assess patient needs, document findings, and collaborate with physicians and the care team.
• Provide bedside palliative care, education, and support to patients and families.
• Triage patient needs based on acuity to prevent avoidable hospitalizations at home.
• Maintain accurate patient records, documentation, and billing per CMS guidelines.
• Collaborate with Medical Director and team to coordinate quality patient care.
• Provide training and continuing education to clinical staff.
• Assist in developing clinical guidelines and standards for high-quality palliative care.
