Job summary

Advanced Home Health & Hospice of St. George is seeking a Medical Social Worker to join our home health and hospice team on a PRN basis, with about 5–10 hours per week and a very flexible schedule.

You will provide emotional and administrative support to clients in private homes and Assisted Living Facilities, handling assessments, referrals, and documentation while maintaining compassionate, patient-centered care.

Qualifications

• Must hold an MSW license in the current state.
• Experience in social work, counseling, or related fields.
• Excellent written and verbal communication skills with the ability to build rapport.

Responsibilities

• Meet with patients per request regarding social services needs.
• Assess nature of clients' situations by interviewing and reviewing personal history.
• Determine milestones to gauge client progress.
• Communicate client updates to all relevant parties.
• Facilitate referrals to other healthcare professionals and programs.
• Maintain accurate and timely client documentation.
