Job summary

Circle of Life Hospice & Palliative Care is seeking a compassionate PRN Hospice Chaplain to provide spiritual and emotional support to patients and families across care settings. You will join an interdisciplinary team, offering prayers, counseling, scripture reading, and culturally sensitive care based on each patient’s wishes.

Travel to patient homes and facilities is required; candidates should demonstrate autonomy, strong communication, and respect for diverse beliefs.

Qualifications

  • Ordination, commissioning, or appropriate authorization from a recognized faith organization preferred.
  • Clinical Pastoral Education (CPE) or hospice chaplain experience preferred.
  • Previous experience providing spiritual care in healthcare, hospice, hospital, or community settings preferred.
  • Strong communication and interpersonal skills.

Responsibilities

  • Provide spiritual and emotional support to hospice patients and their families.
  • Conduct spiritual assessments and identify patient and family spiritual needs.
  • Visit patients in their homes, facilities, and other care settings as needed.
  • Provide prayer, counseling, spiritual guidance, scripture reading, or other appropriate spiritual services based on the patient's wishes.
  • Respect and support patients of all faiths, beliefs, and spiritual backgrounds.
  • Provide support during times of crisis, grief, and end-of-life transition.
  • Assist patients and families with advance care planning and end-of-life concerns when appropriate.
  • Provide bereavement and grief support to patients and families as appropriate.
  • Participate in interdisciplinary team meetings and communicate patient and family needs to the hospice team.
  • Maintain timely and accurate documentation of visits and services provided.
  • Collaborate with nurses, social workers, aides, physicians, and other members of the hospice team.
  • Maintain confidentiality and comply with HIPAA and hospice regulations.
  • Participate in hospice quality improvement and required meetings or training.
