Qualifications

• High School Graduate or GED required.
• Valid Driver License required.

Responsibilities

• Performs personal care for assigned patient(s) and attends to requests promptly.
• Observes patient, reports observations and documents care provided.
• Communicates with patient and family respectfully, maintaining confidentiality.
