Qualifications

• Nursing degree from an accredited college or university.
• Minimum of one (1) year RN experience.
• Current, active, and unrestricted RN licensure in state of practice.
• BSN preferred; acute care experience with adults.
• Home health and/or skilled nursing experience preferred.

Responsibilities

• Provides competent nursing care & interventions to home health patients according to state and industry standards.
• Supervises care provided by Home Health Aides and LPNs on the care team.
• Collaborates with the interdisciplinary team and physician to develop and manage the patient plan of care.
• Performs field visits to evaluate start of care and conduct clinical outcomes monitoring.
• Documents progress using required systems and completes documentation timely at the point of care.
• Maintains patient rights and adheres to physician-approved care plans.
• Ensures safety and regulatory compliance in all nursing activities.
