PruittHealth is seeking a PRN Registered Nurse in Home Health to provide skilled nursing care under physician supervision and coordinate care for patients in the home health setting.

Responsibilities include supervising aides, collaborating with the care team, and ensuring timely documentation and adherence to care plans. Prior RN experience in home health is preferred; BSN is a plus. Union or North Mecklenburg County commutable candidates encouraged.

Qualifikationen

• Minimum RN education: Nursing degree from an accredited college or university.
• Liability insurance to complete home visits.
• BSN preferred and acute care experience beneficial.

• Provides competent nursing care & interventions to home health patients.
• Supervises care provided by Home Health Aides and LPNs.
• Collaborates with the interdisciplinary team to develop and manage the plan of care.
• Demonstrates home health competency and patient care skills.
• Performs field visits to evaluate start of care and monitor outcomes.
• Uses company teaching materials for patient/family education.
• Documents patient progress timely and accurately at the point of care.
