Job summary

Elara Caring in Houston, TX is seeking a Registered Nurse Home Health to join our elite team delivering compassionate care in patients' homes. You will assess needs, perform initial and ongoing evaluations, and implement the plan of care.

This role requires an active RN license, 1+ year of clinical experience, and the ability to lift up to 50 pounds. A driver's license or reliable transportation is required. Home Health experience is a plus.

Qualifications

• Current RN license in state(s) of practice.
• Graduate of accredited Nursing program (Diploma/Associate/Bachelor).
• Minimum 1 year nursing experience in a clinical setting.
• Home Health experience preferred but not required.
• Ability to lift up to 50 lbs and assist with transfers.
• Valid driver's license or reliable transportation to travel between homes.

Responsibilities

• Assess patients to determine care needs.
• Perform initial and ongoing patient assessments.
• Protect patient privacy per HIPAA.
• Provide nursing care per plan of care and physician orders.
• Monitor patient conditions and report changes to the care team.
• Educate patients and families on care and safety.
• Take appropriate action when patient conditions change.
• Help ensure quality, safe, and continuous patient care.
