Job summary

Senior Care Solutions is seeking a PRN Registered Nurse (RN) to provide skilled nursing care in patients' homes within the Melbourne area. The role requires clinical judgment, timely documentation, and adherence to Medicare guidelines.

Responsibilities include SOC/ROC/Recertification visits, comprehensive assessments, care plan development, and patient education. Travel within the coverage area and collaboration with the clinical team are essential. Florida RN license and BLS/CPR required.

Qualifications

• Current, active Florida RN license.
• 1+ year of nursing experience required.
• Home health and Medicare experience preferred.
• OASIS experience preferred.
• Current BLS/CPR Certification.
• Valid Florida driver's license, reliable transportation, and current auto insurance.
• Excellent assessment, communication, and organizational skills.
• Ability to work independently and manage a flexible schedule.

Responsibilities

• Perform SOC, ROC, Recertification, Routine, Evaluation, and Discharge visits.
• Complete comprehensive patient assessments and OASIS documentation when applicable.
• Develop, implement, and update individualized plans of care.
• Administer medications, treatments, wound care, injections, and other skilled nursing services.
• Educate patients and caregivers on disease management, medications, safety, and resources.
• Monitor patient progress and communicate changes to physicians and the team.
• Coordinate with therapists and home health aides to ensure quality care.
• Ensure documentation is accurate and submitted within agency timeframes.
• Maintain compliance with Medicare, state and federal regulations.
