Job summary

Assured Home Health, part of LHC Group within the Optum family, is seeking an experienced RN to provide nursing care in patients' homes across Washington state.

You will coordinate the plan of care with physicians and the health care team, supervise home health aides, complete assessments (OASIS), and ensure compliance with policy, regulations, and quality standards. This role includes on-call responsibilities.

Qualifications

• Current and unrestricted RN licensure in state of Washington.
• Current Driver's License with access to reliable transportation.

Responsibilities

• Initiates and revises the plan of care with physician input and other professionals.
• Supervises home health aides and licensed staff; conducts required visits.
• Provides high-quality nursing care following the plan of care and infection control standards.
• Completes comprehensive assessments (OASIS) and medication reconciliation.
• Documents visits per policy and payer requirements; coordinates with the care team.
• Communicates effectively with physicians, patients, and families to ensure quality care.
