Job summary

Elara Caring is seeking a compassionate PRN Registered Nurse to provide skilled home health care in the Burlington to Andover area. This part-time role offers autonomy, flexibility, and meaningful patient relationships, with 4–12 hours weekly options and mileage reimbursement.

You’ll conduct admissions, visits, and discharges, develop care plans, monitor conditions, educate families, and coordinate with the care team. Massachusetts RN license and travel across communities are required.

Qualifications

• Current, unrestricted Massachusetts RN license.
• Previous home health nursing experience required.
• Ability to work independently and manage visits efficiently.
• Availability to commit to a consistent part-time schedule.
• Valid driver’s license, reliable transportation, and current auto insurance.
• Willingness to travel throughout Bellingham and surrounding communities.
• Ability to sit, stand, bend, lift, and move intermittently and lift 50-100 pounds.

Responsibilities

• Complete admissions, assessments, routine visits, recertifications, and discharges.
• Develop and update individualized plans of care.
• Monitor patients’ conditions and communicate significant changes.
• Coordinate care with physicians, caregivers, and the interdisciplinary team.
• Educate patients and families about medications, conditions, safety, and self-care.
• Complete accurate and timely clinical documentation.
• Help prevent avoidable hospitalizations through proactive nursing care.

Elara Caring is seeking a compassionate PRN Registered Nurse to provide skilled nursing care to patients in their homes. This role is ideal for an RN who understands home health operations, can work independently, and values building meaningful relationships with patients and families. PRN scheduling is available, but dependable and consistent availability is required (whether that is 4 hours/week or 12 hours/week).
