Job summary

Elara Caring is seeking a Home Health Registered Nurse (RN) for a part-time, flexible schedule in Massachusetts. The role focuses on delivering one-on-one, in-home patient care across Bellingham and surrounding communities.

The RN will perform assessments, create and update care plans, and coordinate with physicians and families, ensuring safe, independent home care. Travel within the territory is required and driving is reimbursed during orientation.

Qualifications

• Current, unrestricted Massachusetts Registered Nurse license.
• At least one year of nursing experience in a clinical care setting.
• Home health or home care experience is highly preferred.
• Consistent availability for at least one full 8-hour day each week.
• Valid driver’s license, reliable transportation, and current automobile insurance.
• Ability and willingness to travel throughout the assigned territory.
• Ability to sit, stand, bend, lift, and move intermittently and lift 50–100 pounds.

Responsibilities

• Completing comprehensive nursing assessments.
• Developing and updating individualized plans of care.
• Performing admissions, routine visits, recertifications, and discharges.
• Monitoring patient progress and identifying changes in condition.
• Coordinating care with physicians, caregivers, and interdisciplinary team members.
• Educating patients and families about medications, disease management, and self-care.
• Helping patients remain safe and independent at home.
• Providing proactive interventions that help prevent avoidable hospitalizations.
• Completing accurate and timely clinical documentation.

What You’ll Do
