Job summary

Tri County Home Health Care is seeking a Home Health - PRN - Physical Therapy Assistant (PTA) to deliver in-home PT services and support treatment plans. You will work with physicians and nurses to coordinate care and provide patient/family education on home exercises and care strategies.

The role requires PTA licensure, at least some home health experience, and reliable transportation. You will participate in team meetings, document progress, and adapt plans while maintaining regulatory

Qualifications

PTA certification/license required.
Experience providing physical therapy in a home health setting preferred.
Ability to work independently and as part of a team.
Excellent communication and interpersonal skills.
Strong organizational and time management skills.
Valid driver's license and reliable transportation.
Home Care Home Base experience preferred.

Responsibilities

Provide physical therapy services to patients in their homes.
Assist the physical therapist in developing and implementing treatment plans.
Monitor and document patient progress and adjust treatment plans as needed.
Communicate with physicians, nurses, and other healthcare professionals regarding patient care.
Provide education to patients and their families regarding home exercise programs and other aspects of their care.
Ensure compliance with all regulatory and accreditation standards.
Maintain accurate and complete patient records.
Participate in team meetings and case conferences.
Other duties as assigned.
