Job summary

Tri County Home Health Care is seeking a Home Health Physical Therapy Assistant (PTA) for PRN/home visits in the Uvalde, TX area. The PTA will provide in-home physical therapy services, assist the supervising therapist with treatment plans, and monitor patient progress.

Responsibilities include documenting progress, communicating with the care team, educating patients and families, and ensuring compliance with regulatory standards.

Qualifications

• PTA certification/licensure required.
• Experience providing PT services in home health.
• Ability to work independently and as part of a team.
• Excellent communication and interpersonal skills.
• Strong organizational and time-management skills.
• Valid driver’s license with reliable transportation.
• Home Care Home Base experience preferred.

Responsibilities

• Provide physical therapy services to patients in their homes.
• Assist the PT in developing and implementing treatment plans.
• Monitor and document patient progress and adjust plans as needed.
• Communicate with physicians, nurses and other healthcare professionals.
• Educate patients and families on home exercise programs and care aspects.
• Ensure compliance with regulatory and accreditation standards.
• Maintain accurate and complete patient records.
• Participate in team meetings and case conferences.
