Tri County Home Health is seeking a PRN Physical Therapist to provide in-home PT services. You will assess patients, develop and implement treatment plans, and monitor progress within their homes, collaborating with the care team.

Candidates must hold a current Physical Therapist license in the state, have at least one year of home health experience, and possess strong communication, organizational, and time-management skills. A valid driver’s license and reliable transportation are required.
