Qualifications

• Must possess a valid and current state Physical Therapy license.

Responsibilities

• Provide PRN home health physical therapy to residents on Charlestown campus.
• Implement and monitor residents' personal care plans focusing on function and safety.
• Collaborate with the interdisciplinary clinical team to address residents’ needs.
• Complete documentation to meet reimbursement and regulatory requirements.
