Job summary

Adoration Health is seeking a Home Health Physical Therapist to join our Fort Wayne, IN team on a PRN schedule. You will provide one-on-one patient care in home settings, evaluating and treating patients per physician orders, and coordinating with families and the care team.

The role emphasizes independence with team support, ongoing opportunities, and a focus on compassionate, patient-centered care in a home health environment.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program.
• New physical therapy graduates may be considered in select markets based on program availability.
• At least 2 years of experience as a physical therapist; community/hospice experience preferred.
• Valid license in the state of practice.
• Current CPR certification.

Responsibilities

• Perform physical therapy evaluation and treatment of patients based on physician orders, including recommendations and procurement of adaptive equipment.
• Establish measurable goals and develop treatment plans to move patients toward independence and function.
• Maintain communication among the treatment team, physician, patient, and family members as applicable.
• Maintain thorough and timely documentation of services for proper billing.
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care as allowed by state practice act.
