Adoration Health in Fort Wayne, IN is seeking a dedicated Home Health Physical Therapist to provide patient-centered care in the home setting. You will evaluate, treat, and develop plans to maximize independence, coordinating with physicians, patients, and families.

The role offers a supportive team, flexible scheduling with PRN visits, and opportunities for growth within a growing home health company. Eligible candidates should have a PT license in Indiana, a bachelor's degree in Physical

Qualifications

• Minimum of Bachelor’s Degree in Physical Therapy from an accredited program.
• New graduates may be considered in select markets based on program availability.
• At least 2 years of physical therapy experience; hospice/community experience preferred.
• Valid Indiana license to practice physical therapy.
• Current CPR certification.

Responsibilities

• Perform PT evaluation and treatment based on physician orders, including adaptive equipment.
• Establish measurable goals and develop treatment plans to maximize independence.
• Maintain communication with the treatment team, physician, patient, and family.
• Maintain thorough, timely documentation for billing purposes.
• Supervise assistants and aides in direct patient care per state practice act.

Responsibilities

• P erform physical therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipment
• Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function
• Maintain communication among the treatment team, physician, patient, and family members as applicable
• Maintain thorough and timely documentation of services provided to ensure proper billing
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice act

Qualifications

• Minimum of Bachelors Degree in Physical Therapy from a college or university with accredited Physical Therapy program
• New physical therapy graduates may be considered in select markets based on program availability
• At least (2) years of appropriate experience as a physical therapist. Community/hospice experience is preferred
• Valid license form the state of practice
• Possess and maintains current CPR certification
