Job summary

A leading healthcare provider in the San Francisco area seeks a licensed therapist to develop and implement treatment plans for home health patients. Responsibilities include patient assessment, supervision of assistants, and maintaining documentation. Qualifications

• 2 years of clinical experience as a therapist required.
• Previous home health experience preferred but training available.

• Must be a licensed driver with insured, reliable vehicle.

Responsibilities

• Develop therapy treatment plans for home health patients.
• Implement standardized tests for accurate diagnosis.
• Establish treatment goals and milestones with patients.
• Document treatment plans and progress reports on time.
• Supervise PTAs and COTAs when applicable.
• Communicate patient progress to relevant parties.
• Research related physical and functional problems.

Responsibilities

• Develop therapy treatment plans for home health patients
• Implement standardized tests for accurate diagnosis of physical, cognitive and communication disorders
• Establish treatment goals and milestones with patients
• Timely documentation of treatment plans and progress reports
• For PTs and OTs, be able to supervise PTAs and COTAs
• Update and communicate patient progress to relevant parties
• Research related physical and functional problems

Qualifications

• Current California Professional License required
• 2 years of clinical experience as a therapist is required (including other settings outside of home health)
• Previous experience in home health setting is preferred but we are willing to train
• Possesses and maintains current CPR certification
• Must be a licensed driver with automobile that is insured in accordance with state requirements and is in good working order
