Impact Healthcare LLC is seeking a licensed Physical Therapist to join our Home Health team in San Francisco. In this part-time role, you will evaluate patients at home, design individualized plans, and deliver hands-on therapy to restore strength and mobility.

You will coordinate with physicians, monitor progress, educate families, and adjust treatment as patients improve. Strong knowledge of anatomy, physiology, and kinesiology, plus excellent communication, are essential.

Qualifications

• Current state license as a Physical Therapist.
• At least 1 year of experience in home health or a related field.

Responsibilities

• Conduct thorough evaluations of patients' physical conditions and develop individualized treatment plans.
