Job summary

ExceedCare Group in South San Francisco, CA is seeking licensed therapists to join our home health team, delivering high-quality clinical services in patients' homes across the San Francisco Bay Area.

Responsibilities include developing plans, performing assessments, setting goals, timely documentation, and, for PTs and OTs, supervising assistants. We value independence, flexibility, and strong communication in a supportive environment.

Qualifications

• Licensed therapist with active California license
• Minimum 2 years of clinical therapy experience
• Home health experience preferred but training may be provided
• Current CPR certification required
• Driver with insured vehicle in good working order

Responsibilities

• Develop therapy treatment plans for home health patients
• Implement standardized tests for accurate diagnosis
• Establish treatment goals and milestones with patients
• Timely documentation of treatment plans and progress reports
• Supervise PTAs and COTAs for PT/OT roles as applicable
• Update and communicate patient progress to relevant parties
• Research related physical and functional problems

Responsibilities:

• Develop therapy treatment plans for home health patients
• Implement standardized tests for accurate diagnosis of physical, cognitive and communication disorders
• Establish treatment goals and milestones with patients
• Timely documentation of treatment plans and progress reports
• For PTs and OTs, be able to supervise PTAs and COTAs
• Update and communicate patient progress to relevant parties
• Research related physical and functional problems

Qualifications:

• Current California Professional License required
• 2 years of clinical experience as a therapist is required (including other settings outside of home health)
• Previous experience in home health setting is preferred but we are willing to train
• Possesses and maintains current CPR certification
• Must be a licensed driver with automobile that is insured in accordance with state requirements and is in good working order
