Cleveland Clinic Health System is seeking a PRN Physical Therapist for Homecare in the South Territory. The role covers Medina, Stark, Wayne, Summit and Portage Counties, evaluating and treating patients in their homes.

The position requires driving, home visits, and participation in the hospice interdisciplinary team as needed. A compassionate caregiver with strong assessment and plan-of-care skills is desired.

Formación

• Bachelor’s Degree or certificate.
• Graduate of a Physical Therapy Program accredited by APTA.
• Current State licensure as a Physical Therapist (PT).
• Current BLS Certification through AHA or Red Cross.

Responsabilidades

• Perform initial and ongoing assessments of patient's condition.
• Establish, revise, and evaluate a plan of care with patient/family involvement.
• Perform therapy interventions using standard physical therapy techniques.
• Supervise physical therapy assistants, students, and support personnel.
• Maintain complete and up-to-date documentation of patient condition and care.
• Participate in interdisciplinary assessment of assigned patients.
• Participate on the hospice interdisciplinary team when necessary.

Educación

Bachelor’s Degree or certificate

Graduate of a Physical Therapy Program

Current State licensure as a Physical Therapist (PT)

Current Basic Life Support (BLS) Certification
