The Cleveland Clinic is looking for an experienced Physical Therapist to support home care patients across Medina, Stark, Wayne, Summit and Portage Counties. This PRN position requires flexible hours, weekend rotations, and a 2-week orientation.

You will assess patients in their homes, develop and adjust individualized care plans, perform therapies, supervise staff, and document care while coordinating with the hospice interdisciplinary team.

Competenze

• Minimum qualifications include a PT degree or certificate and Ohio licensure.
• BLS certification required; valid driver's license and auto insurance.
• Experience in physical therapy and home health a plus.

Mansioni

• Perform initial and ongoing assessments of patient condition.
• Establish and revise a care plan with patient and family input.
• Provide therapy interventions appropriate to age and condition.
• Supervise PT assistants and support staff.
• Maintain complete patient documentation and care records.
• Participate in interdisciplinary assessment and hospice team when needed.
