Job summary

Hope Home Care - a LiveWell Partners Network Agency in Ohio seeks a Physical Therapist (PT) to evaluate patients in the home care setting and develop treatment plans. The PT will oversee therapy services, coordinate with physicians and care teams, and ensure proper documentation and discharge planning.

The role requires a PT license, CPR certification, and driver’s license, with opportunities for professional growth in a supportive, patient-centered environment.

Qualifications

• Minimum of 1 year of physical therapy experience; 2+ years in acute or post-acute settings preferred.
• Active state licensure in physical therapy is required.
• Proof of current CPR certification.
• Valid driver’s license.

Responsibilities

• Determine physical therapy treatment plans in consultation with the care team and physicians through initial and ongoing assessments of the patient's condition.
• Provide and complete all proper documentation.
• Administer physical therapy treatments as ordered by a physician; direct and oversee treatment by PT assistants.
• Evaluate effects of treatments and fit of prosthetic/orthotic devices; observe progress and advise adjustments.
• Complete discharge planning with the care team; ensure continuation of therapy post-discharge (home exercise programs, equipment, follow-up).
• Participate in weekly case conferences and communicate about patient care across disciplines.
