Job summary

CommuniCare Family of Companies seeks a dedicated Physical Therapy Assistant (PTA) for its Dignity Care Home Health team in Indianapolis, IN. The PTA delivers patient care under the Physical Therapist's supervision and supports therapy plans in the home setting.

The role requires PTA licensure, a PTA degree, and at least two years of clinical PTA experience. A valid driver’s license and reliable vehicle are also required to travel to patient homes. Competitive benefits offered.

As a Physical Therapy Assistant (PTA), you will deliver patient care under the supervision of the Physical Therapist (PT). Some of your primary responsibilities will include:

• Adhering to and carrying out patient treatment plans under the direction of the Physical Therapist
• Participating in the preparation of clinical and progress notes
• Educating patients and/or caregivers as directed by the PT and patient plan of care
• Establishing and maintaining effective working relationships with team members, supervisors, and the community
• Meeting all service standards for exceptional customer service delivery
• Communicating by telephone or in-person with customers
