Job summary

Logan Health in Kalispell, MT is seeking a Physical Therapist for Home Health on a PRN, part-time basis. The role focuses on developing individualized treatment plans, conducting comprehensive assessments, and delivering high-quality therapy in the patient’s home environment.

The therapist will document care per payer and state guidelines, maintain communication with the care team, and participate in program initiatives to enhance rehabilitation services while meeting productivity expectations.

Qualifications

• Must hold Montana PT license or multi-state compact license for Montana.
• Graduate of an accredited Physical Therapy program and obtain Montana license within 30 days of hire.
• RQI/CPR (BLS) within seven days of hire or before independent patient care.
• Demonstrates integrity, empathy, and strong communication.
• Excellent organizational skills, detail-oriented, and able to work independently or in a team.

Responsibilities

• Develops, implements, and revises individualized PT plans through evaluation and ongoing assessment.
• Performs comprehensive assessments of functional, physical, and psychosocial status.
• Maintains an updated plan of care with goals, interventions, and visit frequency.
• Delivers therapy services according to department standards and policies.
• Completes documentation timely per payer guidelines and Montana practice guidelines.
