Logan Health in Montana is seeking a licensed Physical Therapist to plan and implement individualized treatment programs through comprehensive evaluation and ongoing assessment. The role centers on developing and updating patient care plans within a collaborative team environment.

The ideal candidate holds a Montana PT license (or multi-state compact) and will obtain licensure within 30 days of hire. RQI Provider (BLS) within seven days, with a focus on high-quality outcomes and patient-centered

Qualifications

• MT license or compact license required within 30 days of hire.
• Graduate must obtain MT license or compact license within 30 days of hire.
• RQI Provider (BLS) within seven days of hire/before independent patient care.
• Excellent communication and teamwork in a clinical setting.

Responsibilities

• Develops, implements, and revises individualized, goal-directed physical therapy through evaluation and ongoing assessment.
• Performs comprehensive assessments reflecting patient’s functional, physical, mental, emotional, psychosocial and environmental status.
• Maintains an updated plan of care with goals, interventions and visit frequency.
• Plans, communicates, and supervises to provide therapy services efficiently and competently.
• Completes all treatment per established standards and department procedures.
• Involves in rehabilitation process and conducts admissions, discharge evaluations and daily assessments.
• Ensures timely documentation according to payer guidelines and Montana practice guidelines.
