Qualifications

• Montana Physical Therapy license or multi-state compact license required.
• RQI Provider (BLS) within seven days of hire.
• Excellent verbal and written communication with diverse audiences.

Responsibilities

• Develops, implements, and revises individualized, goal-directed physical therapy through evaluation and ongoing assessment.
• Performs comprehensive assessments reflecting functional, physical, mental, emotional, psychosocial and environmental status.
• Maintains an updated plan of care with goals, interventions, and visit frequency.
• Plans, communicates, supervises and delegates to provide therapy services efficiently.
• Delivers all patient treatment per established physical therapy standards and procedures.
• Completes admissions, discharge evaluations, daily assessments and sets patient goals.
• Provides excellent customer service and communication with internal and external customers.
• Completes all documentation per payer guidelines and Montana practice guidelines.
