Job summary

Elara Caring is seeking a Medical Social Worker (MSW) to assess psychosocial needs, develop care plans, and coordinate resources for patients across home-based care settings.

The MSW will collaborate with physicians and the care team to ensure seamless discharge planning, educate families, and support positive patient outcomes. A state social work license and CSWE-accredited MSW/DSW are required, plus healthcare experience and travel flexibility.

Qualifications

• Current State License as a Social Worker.
• Master’s or Doctoral degree in Social Work from CSWE-accredited school.
• 1+ year of social work experience in a healthcare setting.
• Social work skills per general professional standards.
• Ability to frequently lift, push, pull, and support up to 50 pounds.
• Must possess a valid driver’s license or have reliable transportation to travel between patient homes.

Responsibilities

• Assess patients to identify psychosocial, financial, environmental, and safety needs.
• Complete initial social work evaluations and reassess needs during follow-up visits.
• Develop and update individualized care plans based on patient needs and physician orders.
• Communicate changes in patient condition to physicians and care team.
• Connect patients and families with community resources and support services.
• Counsel and educate patients and families to address social, emotional, and care-related needs.
• Coordinate with care team to support continuity of care and discharge planning.
• Provide compassionate, high-quality social work services to support positive patient outcomes.

As a Medical Social Worker (MSW)

As a Medical Social Worker (MSW), you’ll contribute to our success in the following ways:

What is Required?

• Current State License as a Social Worker
• Master’s Degree or Doctoral Degree in Social Work from a school of Social Work accredited by the Council of Social Work Education
• 1+ year of social work experience in a healthcare setting
• Social work skills as defined as general social work standards
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
