Job summary

Elara Caring is seeking a Medical Social Worker (MSW) in Missouri to provide psychosocial assessments, care planning, and family education in patients' homes. You will work with a care team to ensure safe discharge and continuity of care, using community resources to support patients’ goals.

Requires an active State Social Work license and an MSW/DSW degree from a CSWE-accredited school, with at least one year in a healthcare setting.

• 1+ year of social work experience in a healthcare setting.

Responsibilities

• Assess patients to identify psychosocial, financial, environmental, and safety needs.

• Develop and update individualized care plans based on patient needs and physician orders.
• Communicate changes in patient condition to physicians and the care team.
• Connect patients and families with community resources and support services.
• Counsel and educate patients and families on social and care needs.
• Coordinate with the care team to support discharge planning and continuity of care.
• Provide high-quality social work services to support positive patient outcomes.

As a Medical Social Worker (MSW), you’ll contribute to our success in the following ways:

• Provide compassionate, high-quality social work services that support positive patient outcomes.

What is Required?

• Current State License as a Social Worker
• Master's Degree or Doctoral Degree in Social Work from a school of Social Work accredited by the Council of Social Work Education
• 1+ year of social work experience in a healthcare setting
• Social work skills as defined as general social work standards
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations
