Elara Caring is seeking a Medical Social Worker (MSW) to evaluate patients’ psychosocial needs, develop care plans, and coordinate with the care team for safe, effective discharges. The role requires a current social work license and an MSW/DSW degree, with at least one year in healthcare social work.

The MSW will connect patients and families to community resources, provide counseling, and support continuity of care across home-based services.

Qualifications

• Current State License as a Social Worker.
• Master’s Degree or Doctoral Degree in Social Work from a CSWE‑accredited program.
• 1+ year of social work experience in a healthcare setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds.
• Must possess a valid driver’s license or reliable transportation to travel to patient homes.

• Develop and update individualized care plans based on patient needs, goals, and physician orders.

What is Required?

• Current State License as a Social Worker
• Master’s Degree or Doctoral Degree in Social Work from a school of Social Work accredited by the Council of Social Work Education
• 1+ year of social work experience in a healthcare setting
• Social work skills as defined as general social work standards
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver’s license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
