Job summary

Elara Caring is seeking a Medical Social Worker (MSW) to support patients in their homes in Texas. You will assess psychosocial needs, develop care plans, and coordinate with the care team to ensure safe discharges and continuity of care.

The role requires an MSW (or higher) and licensure, with at least one year in healthcare social work. Expect a comprehensive benefits package and a driver’s license for home visits.

As a Medical Social Worker (MSW), you’ll contribute to our success in the following ways:

What is Required?

• Current State License as a Social Worker
• Master’s Degree or Doctoral Degree in Social Work from a school of Social Work accredited by the Council of Social Work Education
• 1+ year of social work experience in a healthcare setting
• Social work skills as defined as general social work standards
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
