Job summary

Living Hope Home HealthCare in Houston seeks an experienced Registered Nurse to join our home health team. You will work one-on-one with patients in their homes, provide skilled nursing care per physician orders, and coordinate with LVNs and home health aides to ensure comprehensive patient care.

The role requires Texas RN licensure, CPR/BLS, and 1 year of home health experience. Expect a competitive salary, paid training, and a strong team environment with opportunities for growth.

Qualifications

• Must hold Texas RN license.
• BLS certification required.
• 1 year Home Health experience preferred.
• CPR certification required.
• Strong documentation skills and independent problem solving.
• Excellent interpersonal skills and critical thinking.

Responsibilities

• Provide skilled nursing care in patients' homes per physician orders and directives to staff.
• Perform patient admissions, develop and update plans of care, and manage recertifications and discharges.
• Coordinate with interdisciplinary team and participate in weekly case conferences.
• Document care with computerized systems and ensure accurate recordkeeping.
• Travel to Houston and surrounding areas as required (100%).
