Job summary

Chemik Healthcare Services is seeking Registered Nurses for home health in Houston and surrounding areas. The role involves Oasis to admit, transfer, re-certify, and discharge patients, coordinating LVNs and HHAs, and ensuring compliance with Medicare/Medicaid regulations.

The position offers flexible scheduling with both part-time and full-time options. Training is available, and CradleMRX EMR experience is preferred.

Qualifications

Be a registered nurse with a current license.
Minimum of one year of nursing experience as an RN.
Work positively and favorably with patients, families, and staff.

Responsibilities

Perform the initial home care patient visit and re-evaluate the patient’s needs and progress on a regular basis.
Initiate the plan of care under doctor’s orders.
Perform admission, transfer, re-certification, resumption of care, and discharge Oasis for the home care patient.
Observe, assess, and document symptoms.
Educate patients and caregivers on disease process, medications, plan of care, and individualized treatment plans.
Coordinate patient services.
Supervise LVNs and HHAs.
Notify the physician and other personnel of change in the patient’s condition.
Discharge the patient when discharge criteria are met.
Update clinical records according to policy and procedures.
Provide onsite supervision of LPN/HHA.
