Job summary

ComForCare Home Care – North Austin is seeking an experienced PRN Registered Nurse (RN) with a background in home care and geriatric care to perform client assessments, start-of-care visits, RN delegation, education, care coordination, and clinical support in clients' homes. This is a PRN/as-needed role, not a traditional bedside or PDN position.

Qualifications

• Current, unrestricted Texas RN license.
• Home care or home health experience required.
• Geriatric/senior populations experience required.
• Knowledge of Texas RN delegation strongly preferred.

Responsibilities

• Conduct initial client assessments and start-of-care visits as needed.
• Develop, review, and update individualized care plans.
• Perform RN delegation, including caregiver instruction, competency validation, supervision, and documentation.
• Assess whether client-specific tasks are appropriate for delegation and provide follow-up and oversight.
• Educate clients, families, and caregivers regarding care needs, safety, fall prevention, mobility, nutrition, medication awareness, and aging safely at home.
• Identify changes in client condition or needs and communicate appropriate recommendations and next steps.
• Conduct reassessments, supervisory visits, and other nursing visits as needed.
• Coordinate with families, caregivers, physicians, home health, hospice, therapy providers, and other healthcare professionals as appropriate.
• Accompany clients to medical appointments when requested, helping facilitate communication and understanding of follow-up instructions.
• Support clients and families during changes or transitions in care.
• Maintain accurate and timely documentation.
• Serve as a clinical resource and educator for caregivers and the home care team.
