Job summary
Overture Home Care LLC in Dallas is seeking a compassionate Registered Nurse for a PRN role to perform in-home and facility-based client assessments in the Dallas area. Qualifications
- Must hold a full, active RN license.
- Minimum of two years of geriatric healthcare experience.
- Ability to travel within the Dallas area and adapt to flexible scheduling.
- Strong verbal and written communication.
Responsibilities
- Conduct in-home and facility-based client assessments to determine care needs.
- Collaborate with intake and nursing teams to ensure care coordination.
- Evaluate client conditions and recommend levels of care.
- Maintain accurate documentation of assessments and findings.
- Communicate with clients, families, and referral sources to support plans.
- Ensure compliance with state regulations and company policies.
- Support care transitions from hospitals and rehab facilities.
- Build relationships with referral partners and healthcare providers.
- Provide clinical insight to nursing and caregiving staff.
- Participate in case reviews and quality improvement initiatives.
- Travel within the Dallas area as required.
