Job summary

Overture Home Care in Dallas is seeking a compassionate Registered Nurse for PRN in-home assessments of senior clients. You will collaborate with care teams, document findings, and determine care levels, with an emphasis on Alzheimer's, dementia, hospice, and geriatric care.

The role requires a current RN license and at least two years' geriatric experience; travel within the Dallas area is expected. Flexible scheduling and responsiveness to client needs are valued.

Qualifications

• Must hold a full, active RN license.
• Minimum of two years’ related geriatric healthcare experience.
• Excellent written and verbal communication skills.
• Ability to work independently and manage time effectively.
• Proven competency in documenting care and clinical findings.

Responsibilities

• Perform in-home and facility-based client assessments to determine care needs and eligibility.
• Collaborate with intake, case management, and nursing teams for care coordination.
• Evaluate client conditions and recommend appropriate levels of care.
• Document all assessments and clinical findings accurately and timely.
• Communicate with clients, families, and referral sources to support care planning.
• Ensure compliance with state regulations and company policies during assessments.
• Support care transitions from hospitals or rehab facilities.
• Build relationships with referral partners and healthcare providers.
• Provide clinical insights and support nursing and caregiving staff as needed.
• Participate in case reviews and quality improvement initiatives.
• Travel within the Dallas area to complete assessments.
