Hayward Gardens Post Acute in Hayward, CA is seeking a Certified Occupational Therapy Assistant (COTA) to join our rehabilitation team on a PRN basis, working under the supervision of a licensed Occupational Therapist to provide hands-on treatment for residents in a skilled nursing setting.

The role focuses on implementing treatment plans, accurate documentation, and collaboration with the interdisciplinary team to achieve optimal rehabilitation outcomes and improve residents’ quality of life.

Responsabilités

• Provide occupational therapy treatments as directed by the supervising Occupational Therapist.
• Complete timely, accurate, and compliant clinical documentation in accordance with facility, state, federal, and payer requirements.
• Maintain a safe treatment environment by following infection control, safety, and equipment maintenance protocols.
• Participate in Resident Care Conferences and rehabilitation meetings as needed.
• Assist the nursing department with training Restorative Aides.
• Communicate regularly with the supervising Occupational Therapist regarding resident progress, treatment goals, physician updates, and discharge planning.
