Job summary

AdvisaCare Hospice is hiring a compassionate RN to join our Troy-based team servicing Wayne/Monroe counties. You will provide home-based hospice care, conduct supervisory visits, and guide an interdisciplinary group to ensure high-quality patient care.

The role emphasizes family education, on-call availability, and adherence to policies while delivering dignified end-of-life support in a fast-paced environment.

Qualifications

• Current Michigan license in good standing.
• Minimum 1 year hospice experience.
• Empathetic approach to patients and caregivers.
• Full-time on-call availability.

Responsibilities

• Initial and ongoing assessment of patient’s physical, functional, psychosocial and environmental needs.
• Implement the individualized plan of care and revisions as needed.
• Compliance with AdvisaCare Hospice Policies and Medicare requirements.
• Educate patient/family on disease process, self-care, end-of-life care and ethical considerations.
• Supervision of paraprofessionals providing services to patient.
• Prepare clinical and progress notes; participate in IDG conferences.
• Participate in Hospice performance improvement program.
• Maintain high level of customer satisfaction with clients, families and caregivers.
• Communicate information using current process and technology within the organization.

We are a “Successful” and “GROWING” fast paced Hospice Agency.

Are you looking for a challenging and rewarding position as a PRN/CONTINGENT Hospice RN? We are searching for compassionate, dynamic, energetic, organized and efficient professionals with Hospice experience to become part of our Hospice Family with our Troy office location being your Parent Agency servicing our clients for the Wayne and Monroe County.

For this role, you need to be a registered nurse who sees the value of Hospice care. You will be providing emotional support, with the goal to help patients live their final days with dignity and comfort. Our Hospice RN’s will conduct appropriate supervisory visits which allow for clinical observation of performance of assigned team members. Participate and provide feedback to the team for regular interdisciplinary group meetings; on the development, review and revision of the patient/family plan of care.

Requirements

• Ability to adapt to patient’s status and needs on a full time on call basis
