Job summary

AdvisaCare is seeking a compassionate Hospice RN to join our team and deliver one-on-one care in patients' familiar surroundings in the Lansing area. This full-time role emphasizes building relationships and individualized care plans with a patient- and family-centered approach.

Responsibilities include comprehensive assessments, care plan implementation, and interprofessional collaboration to ensure dignity and comfort at end of life. Field visits and on-call assignments may be required.

Qualifications

• Current Michigan License in good standing.
• Minimum 1 year hospice experience.
• Empathetic approach to patients and caregivers.
• Participate in field visits daily and weekly office meetings as indicated.
• Ability to adapt to patient status and needs on a full-time on-call basis.

Responsibilities

• Initial and ongoing assessment of patient needs (physical, functional, psychosocial, environmental).
• Implement the individualized plan of care and revisions as needed.
• Ensure compliance with hospice policies and Medicare requirements.
• Educate patient/family on disease processes, self-care, and end-of-life care.
• Supervise paraprofessionals providing services to patients.
• Prepare clinical notes and participate in IDG conferences.
• Participate in hospice performance improvement programs.
• Maintain high levels of customer satisfaction with clients, families, and caregivers.
• Communicate information using current processes and technology within the organization.

Are you looking for a challenging and rewarding position as a Full time Hospice RN? We are searching for compassionate, dynamic, energetic, organized and efficient professionals with Hospice experience to become part of our Hospice Family with our Troy office location being your Parent Agency servicing our clients for the Lansing and surrounding areas!

For this role, you need to be a registered nurse who sees the value of Hospice care. You will be providing emotional support, with the goal to help patients live their final days with dignity and comfort. Our Hospice RN's will conduct appropriate supervisory visits which allow for clinical observation of performance of assigned team members. Participate and provide feedback to the team for regular interdisciplinary group meetings; on the development, review and revision of the patient/family plan of care.
