Job summary

AdvisaCare is expanding its Hospice services from our Livonia office, servicing Wayne and Monroe counties. We are seeking compassionate Hospice RN professionals for PRN/contingent roles with Troy as the parent office.

You will provide emotional support, conduct supervisory visits, and contribute to patient/family plans of care. Responsibilities include patient assessments, plan implementation, and interdisciplinary collaboration while ensuring Medicare compliance and high patient satisfaction.

Qualifications

• Current Michigan RN License in good standing.
• Minimum 1 year hospice experience.
• Empathetic approach to patients and caregivers.

Responsibilities

• Initial and ongoing assessment of patient’s physical, functional, psychosocial and environmental needs.
• Implement the individualized plan of care and revisions as needed.
• Compliance with AdvisaCare Hospice Policies and Procedures, Medicare compliance.
• Educate patient/family on disease process, self-care, end-of-life care, ethical concerns.
• Supervision of paraprofessionals providing services to patient.
• Prepare clinical and progress notes, participate in IDG conferences.
• Participate in Hospice performance improvement program.
• Maintain high level of customer satisfaction with clients, families and caregivers.
• Communicate information using current process and technology within organization.

Are you looking for a challenging and rewarding position as a PRN/CONTINGENT Hospice RN's (Registered Nurse)? We are searching for compassionate, dynamic, energetic, organized and efficient professionals with Hospice experience to become part of our Hospice Family with our Troy office location being your Parent Agency servicing our clients for the Wayne and Monroe County.

For this role, you need to be a registered nurse who sees the value of Hospice care. You will be providing emotional support, with the goal to help patients live their final days with dignity and comfort. Our Hospice RN's will conduct appropriate supervisory visits which allow for clinical observation of performance of assigned team members. Participate and provide feedback to the team for regular interdisciplinary group meetings; on the development, review and revision of the patient/family plan of care.

Requirements:

• Ability to adapt to patient’s status and needs on a full time on call basis
