Job summary

AdvisaCare is seeking a compassionate Licensed Practical Nurse (LPN) to join our hospice team on a PRN/contingent basis in Michigan. You will assess patient needs, implement care plans, and collaborate with interdisciplinary teams to provide dignified end-of-life care.

Qualifications

• Current Michigan LPN license in good standing.
• Minimum of 1 year experience in hospice or palliative care.

Responsibilities

• Conduct initial and ongoing assessments of patient needs.
• Implement and adjust individualized care plans.
• Ensure compliance with hospice policies and Medicare regulations.
• Educate patients and families on self-care and end-of-life options.
• Supervise paraprofessionals delivering patient care.
• Document and report clinical findings and participate in interdisciplinary meetings.
• Enhance patient satisfaction and experience.
